Real-time CT Fluoroscopy Guided Automated Needle Biopsy of Pulmonary Lesions: Analysis of Successful Biopsy Rate and Frequency of Pneumothorax

We evaulate the rate of successful biopsy and multiple variables on the rate of post-biopsy pneumothorax using 18-gauge cutting needle under real-time CT fluoroscopy guidance. One hundred thirteen patients with focal lung lesion who had undergone CT fluoroscopy guided lung biopsy were retrospectively reviewed. We evaluated the rate of successful biopsy and pneumothorax rate. In case in which the pneumothorax was present, the following variables were analyzed; 1) size of pulmonary lesion; 2) biopsy site; 3) distance of pulmonary lesion from pleura; 4) emphysema; 5) pleural thickening at the biopsy site. Biopsy was successful in 103 of 113 patients (91%) and pneumothorax occured at 25 of 113 (22%). An increased rate of pneumothorax was correlated with distance of pulmonary lesion from pleura (p=0.003). Size, biopsy site, emphysema, and pleural thickening at the biopsy site were not correlated with pneumothorax rate. 18-gauge cutting needle under real-time CT fluoroscopy guidance is useful biopsy procedure more safely, and more conveniently, comparable to previously noted procedure.
